In consideration of the services of Adantiah LLC, d.b.a Ascentiah Healing Center , its agents, owners, participants, employees, and all other persons or entities acting in any capacity on its behalf (hereinafter collectively referred to as ASCENTIAH HEALING CENTER , I hereby agree to release and discharge ASCENTIAH HEALING CENTER , on behalf of myself, my children, my parents, my heirs, assigns, personal representative and estate as follows:

1. I understand that the treatment and therapies rendered or recommended by ASCENTIAH HEALING CENTER do not treat or diagnose dis-ease, and these therapies cannot be replaced with medical treatments. The Practitioner will work with you using his/her own unique selections of energy healing modalities as well as other therapeutic techniques that each individual practitioner may incorporate with. If you are not comfortable with any specific modalities and do not want them used in sessions with you, please notify the Practitioner right away.

2. I understand that the healing modalities used by your Practitioner are not intended, and should not be used, to diagnose, treat, or cure any medical or psychological ailment. Additionally, you understand that these modalities are not a substitute for medical, psychological, or therapeutic advice from a licensed health care professional. You understand and acknowledge that the Practitioner’s statements and opinions are theirs alone and any information provided to you is for informational purposes only.

3. I understand that my participation in healing sessions with the Practitioner does not create a physician-patient or therapist-patient relationship. I am solely responsible for consulting with a licensed physician or mental health professional if I require physical or mental health care or if I desire to use, or stop using, any prescription medication. I also agree that I will not hold the Practitioner liable for any harm that may result from my failure to consult with a licensed physician or mental health professional or my failure to follow such professional’s advice.

4. I hereby agree that I will not hold the Practitioner liable for any injury, illness, damages, harm, or losses of any kind that you might incur as a direct or indirect result of you purchasing or participating in healing sessions. I further agree to indemnify and defend the Practitioner against any claims made by any person other than myself for damages that I may cause as a direct or indirect result of you purchasing or participating in healing sessions. This waiver and indemnity extend to any business entity listed at the beginning of this document, along with that entity’s owners, officers, directors, employees, agents, successors, and assigns.

5. I understand fully that there is no guarantee that the advice given during a session may not give me the desired outcome. I will not hold ASCENTIAH HEALING CENTER and its officers responsible for any harm or lack of results while receiving their advice. I hereby release ASCENTIAH HEALING CENTER from any and all liability now and in the future that may occur as a result of following any or all the advice given.

6. I acknowledge that I am receiving advice for myself or on behalf of my family member, and that it is my choice to adhere to or reject the provided advice. My participation and follow through on any advice is voluntary, and by signing this waiver I accept responsibility for any harm or injury that may result from my participation in the wellness program.

7. Any advice regarding dietary supplements, water or any other items sold, provided by or recommended by ASCENTIAH HEALING CENTER is done so by opinion only, and these products may not have been approved by the FDA. I acknowledge that ASCENTIAH HEALING CENTER is not liable for any negative repercussions.

8. By agreeing to these terms I am accepting that I will not hold ASCENTIAH HEALING CENTER accountable for any issues, health related or non‐health related that may result from consuming a product suggested or recommended by ASCENTIAH HEALING CENTER. I understand that I am responsible for understanding any health risks involved in consuming a dietary supplement, water or any other item sold in ASCENTIAH HEALING CENTER.

9. I have read this liability waiver and assumption of risk and fully understand its terms. I acknowledge that I am signing the agreement freely and voluntarily and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law in the State of New York.
